Resumo: Elongated uvula is an uncommon anatomical variation that may lead to clinical manifestations such as local discomfort and respiratory impairments. This report describes the case of a 45-year-old female patient with a lifelong elongated uvula, who noticed progressive enlargement at its distal portion over the past four years. The patient reported dysphagia, xerostomia, and respiratory dysfunctions, particularly during sleep. Clinical examination revealed an elongated palatine uvula, with nodular and irregular thickening at its tip. Due to the negative impact on quality of life, partial uvulectomy was indicated and performed, resulting in symptom relief. Histopathological analysis revealed hyperplastic tissue associated with oral squamous papilloma, characterizing a benign proliferative lesion associated with the anatomical variation. The surgical procedure and postoperative course were satisfactory, highlighting the effectiveness of uvulectomy in the management of benign oropharyngeal lesions and the essential role of histopathological examination in the diagnosis of proliferative changes in the oropharynx.
