Resumo: A 47-year-old female patient presented with painful ulcerated and blistering lesions affecting the oralmucosa and skin, with a five-month duration, suggestive of an autoimmune disorder. Initially, thelesions were localized with lower morbidity; however, the condition progressed. Given the lack ofresponse to systemic corticosteroid therapy and the severity of the clinical presentation, characterizedby a 48-hour period of fever and without oral intake, the patient was hospitalized for clinicalstabilization and subsequently underwent a biopsy. Histopathological analysis of a periumbilical skinlesion revealed suprabasal acantholytic bullous dermatosis, consistent with pemphigus vulgaris.Treatment with high doses of corticosteroids and immunosuppressants resulted in clinicalimprovement. The patient has been under follow-up for one year, showing partial remission. Earlydiagnosis of pemphigus vulgaris allows for timely initiation of therapy, helping to prevent diseasedissemination and reducing the risk of mortality due to infections and treatment-related complications.
