Resumo: Unicystic ameloblastoma is an uncommon odontogenic tumor in early childhood, requiring accurate imaging for proper diagnosis and treatment planning. This case report describes a 3-year-old female patient presenting with swelling in the anterior maxilla. Panoramic radiograph revealed a well-defined radiolucent lesion, leading to cone-beam computed tomography (CBCT). CBCT Imaging demonstrated a large hypodense, unilocular, corticated lesion associated with the developing permanent left central incisor, extending posteriorly to the region of the deciduous second molar. CBCT also revealed buccal/palatal cortical expansion and thinning, with superior displacement of the nasal cavity floor, left maxillary sinus, and adjacent teeth. The differential diagnosis included adenomatoid odontogenic tumor, dentigerous cyst, and unicystic ameloblastoma. Histopathological analysis confirmed unicystic ameloblastoma. A decompression device was installed, and the patient remains under clinical and radiographic monitoring. The therapeutic goal is lesion regression to allow a more conservative surgical approach, preserving maxillofacial structures critical to the patient's growth and development.
