Resumo: Dysosteosclerosis is a rare genetic disorder classified among osteosclerotic metabolic diseases and is frequently misdiagnosed due to its clinical similarities with osteopetrosis despite the limited number of cases reported in the literature. It is characterized by recurrent fractures, failure of dental eruption, short stature, neurological impairment resulting from nerve compression, and skin abnormalities. Osteomyelitis is a prevalent condition and contributes to poor prognosis. We present the case of a 53-year-old male diagnosed with dysosteosclerosis, exhibiting significant maxillofacial manifestations. The patient underwent multiple surgical interventions, including total mandibulectomy, to address recurrent jaw osteomyelitis. In 2019, he developed osteomyelitis with an associated oroantral fistula, necessitating several corrective surgeries. Despite treatment efforts, the fistula recurred and is currently accompanied by infectious dacryocystitis. This case illustrates the clinical challenges in managing this rare disease and highlights the importance of multidisciplinary approaches to enhance the patients quality of life despite an uncertain prognosis.
