Resumo: A 15-year-old male presented to our Oral Diagnosis Clinic with a painless facial swelling that had developed over six months. Clinical examination revealed facial asymmetry due to left maxillary expansion without skin changes. Intraorally, the upper left anterior alveolar ridge appeared rounded and swollen, mainly on the vestibular side, with slight palatal involvement and cortical bone loss. There was no tenderness, tooth displacement, or mucosal alteration. CT images showed a unilocular radiolucent lesion with peripheral calcifications and a tooth-like structure at its outer margin. A provisional diagnosis of calcifying odontogenic cyst (Gorlin cyst) was made. Marsupialization and biopsy were planned due to lesion size. Aspiration yielded translucent fluid. Histopathological analysis confirmed the diagnosis, revealing a cystic lesion lined with stratified epithelium, cuboidal basal cells, numerous ghost cells, and calcifications. The patient was followed post-operatively and experienced a smooth and complete recovery.
