Resumo: HPV-related sinonasal multiphenotypic carcinoma is a rare entity with distinctive morphological and immunohistochemical features. A 46-year-old male patient presented with persistent nasal obstruction refractory to clinical treatment. Radiological evaluation revealed a right maxillary sinus tumor with extension to the maxilla. Incisional biopsy demonstrated a multiphasic morphology, displaying features of both surface carcinoma and minor salivary gland carcinoma, with basaloid cell nests and sheets surrounding microcystic spaces containing mucoid material, mimicking adenoid cystic carcinoma, as well as glandular formations within basaloid cell nests. Comprehensive immunohistochemical analysis showed positivity for AE1/AE3, CK-7, calponin, SOX-10, LEF-1, and p16, while MYB, beta-catenin, and CK5/6 were negative. HPV genotyping by PCR was positive for HPV33. The final diagnosis was HPV-Related Sinonasal Multiphenotypic Carcinoma. The patient underwent complete surgical resection with clear margins. At 16 months of rigorous clinical and radiological follow-up, the patient remains disease-free with no evidence of local recurrence or distant metastasis.
