Título: HPV GENOTYPING AND CLINICOPATHOLOGICAL ANALYSIS OF OROPHARYNGEAL CARCINOMAS IN A BRAZILIAN COHORT

Resumo: Objective: To characterize the clinicopathological features and HPV genotype distribution of oropharyngeal and base of tongue squamous cell carcinomas in a population from Northeast Brazil.
Study Design: Retrospective analysis of 108 patients with oropharyngeal squamous cell carcinomas diagnosed between 2015-2021. HPV status was determined by molecular testing, with additional p16 and p53 immunohistochemistry. Demographic data, histopathological features, TNM staging, and clinical outcomes were analyzed.
Results: HPV positivity was observed in 44.4% (48/108) of cases. HPV-positive patients were younger (mean age 51.2 vs 60.3 years), predominantly male (70.8%), and showed less association with smoking (31.3% vs 76.7%) and alcohol consumption (25.0% vs 60.0%). HPV16 was the predominant genotype (85.4%), followed by HPV33 (8.3%). Histologically, HPV-positive tumors were predominantly basaloid (50.0%) and non-keratinizing (33.3%). Most HPV-positive cases presented at stage III (50.0%). p16 positivity strongly correlated with HPV status (93.8%). The recurrence rate was lower in HPV-positive patients (35.4% vs 46.7%), increasing significantly with higher TNM stages.
Conclusion: This study confirms previous findings that HPV-associated oropharyngeal carcinomas affect younger patients, show less association with smoking, and have better prognosis. The strong correlation between p16 and HPV positivity supports p16 as a surrogate marker where HPV testing is unavailable.
