Resumo: Odontogenic maxillary sinusitis is an inflammatory process of the maxillary sinuses caused by odontogenic infections related to dental pathologies or procedures. Most cases are due to iatrogenic factors such as oroantral fistulas resulting from tooth extractions, endodontic treatments, or failed dental implants. Many patients are asymptomatic, with diagnosis commonly based on tomographic findings. This case report describes a 68-year-old male patient diagnosed with odontogenic maxillary sinusitis during hospitalization in an intensive care unit. Tomography revealed odontogenic material and secretion within the maxillary sinus. Treatment included antibiotic therapy, nasal irrigation, and surgical intervention. Surgery involved creating a bony window to access the sinus, allowing fragment removal and curettage. Follow-up imaging showed improvement in maxillary sinus opacification. Early recognition and proper management of odontogenic maxillary sinusitis are essential to prevent complications, such as infection spread, and to contribute to better clinical outcomes.
