Título: MALIGNANT PROGRESSION IN SALIVARY GLAND PLEOMORPHIC ADENOMA: A SYSTEMATIC REVIEW

Resumo: Objective: To evaluate the malignant transformation of pleomorphic adenoma (PA) into carcinoma ex-pleomorphic adenoma (CxPA), focusing on transformation rate, risk factors, and demographic data.
Study design: A systematic review following PRISMA guidelines was conducted. Searches in PubMed, Scopus, Web of Science, and Embase identified studies on PA malignization, with no restrictions on publication date or language. Inclusion criteria were cohort studies, case series (≥50 cases), and clinical trials reporting malignization during follow-up. Excluded were conference abstracts, literature reviews, small case series, and studies not reporting relevant data. Twenty studies were included, and data on study design, patient characteristics, location, clinical presentation, duration, and recurrence were extracted. Study quality was assessed using the JBI tool for systematic reviews.
Results: Most PA patients are female, but malignancy is more common in males. Diagnosis typically occurs in the fifth and sixth decades of life, while younger patients have higher recurrence rate. The parotid gland is the most common location, with swelling being the most frequent symptom. The recurrence rate is 17.6%, increasing with more recurrences. Malignant transformation occurs in about 3% of cases, also increasing with recurrences.
Conclusion: Although rare, the malignant transformation of PA is more likely with recurrence, particularly in patients with long-term follow-up.
