Resumo: Female patient, 7 years old, with extensive VM involving the oral cavity, tongue, floor of the mouth, oropharynx, cervical region, and right auricle. Under follow-up with the plastic surgery team (PST) and requiring dental treatment to remove infectious foci in close contact with the VM. After CT and MRI, the PST found no indication for prior embolization. She underwent the procedure under general anesthesia and, during electrocauterization of hyperplasia and initiation of syndemostomy, presented with high-flow bleeding unresponsive to local measures. The interventional radiology team was called; the first embolization failed to control the bleeding, and a second attempt reduced the flow but did not stop it. A fibrin sealant and surgicel dressing was applied for 72 hours. The patient remained hemodynamically stable, but hypotensive and tachycardic, it was transferred to the ICU. After seven days, it was possible to safely remove the infectious foci with local hemostatic control.
