Resumo: Oral squamous cell carcinoma (OSCC) incidence is rising among non-smokers/non-drinkers (NSND). We report five NSND cases (4 females, 26-52 years), with tongue (4/5) and gingival (1/5) lesions linked to prior trauma. Clinical presentations included erythroleukoplakic erosions, exophytic nodules, and crateriform ulcers. Mean symptom duration was 7 months (3-12), with delayed diagnosis due to initial misclassification as traumatic/inflammatory lesions. Histopathology confirmed OSCC (T1-T3) with significant stromal invasion (all cases) and perineural involvement (2/5). Diagnostic delays necessitated aggressive therapies (surgery + radiotherapy in 3/5 cases). These findings highlight the need for heightened clinical suspicion of OSCC in NSND patients, particularly women with tongue lesions and trauma history. Early recognition of atypical presentations (e.g., persistent erythroleukoplakia) in low-risk groups is essential. Interdisciplinary training for dentists and physicians should emphasize OSCC differentials in NSND populations, enhancing early detection and multidisciplinary collaboration to reduce delays and improve prognosis.
