Resumo: Chronic osteomyelitis is an inflammatory condition affecting the gnathic bones, often caused by odontogenic bacteria, presenting with edema, trismus, and suppuration, rarely associated with cemento-osseous dysplasia. A 58-year-old female presented with a right molar region bone sequestrum and suppuration following trauma two years prior. Initial management included clindamycin 300 mg for 10 days and 0.12% chlorhexidine mouthwash for 14 days. Imaging confirmed necrotic bone and hyperdense images with hypodense margin, prompting surgical sequestrectomy and curettage. Histopathology revealed necrotic bone with lymphoplasmacytic infiltrate, mineralized basophilic spherules, and connective tissue with the presence of dysplastic bone tissue deposition and cementous-like material. After 4 years of follow-up, the patient remains asymptomatic with no recurrence. This case highlights the efficacy of combined antibiotic therapy and surgical debridement in resolving chronic osteomyelitis in an area of dysplastic bone tissue, emphasizing the importance of long-term monitoring.
