Resumo: Amyloidosis is a rare disease that may present oral manifestations, particularly on the tongue, and can be an early sign of systemic involvement. A 63-year-old female was referred for evaluation of a lesion on the dorsal surface of the tongue. Clinical examination revealed a smooth, erythematous plaque with papillary atrophy and a single submucosal nodule measuring approximately 0.6 × 0.9 cm. The initial diagnosis was median rhomboid glossitis, and the patient was treated with topical and systemic antifungals. Due to lack of clinical improvement, an excisional biopsy of the nodule was performed. Histopathological examination revealed eosinophilic, amorphous material in the connective tissue, and Congo red staining confirmed the presence of amyloid deposits. A diagnosis of amyloidosis was established, and the patient was referred for systemic evaluation, which confirmed systemic amyloidosis. Unfortunately, the patient died two months later due to disease complications. This case highlights the relevance of oral findings in the early diagnosis of systemic conditions.
