Resumo: Mammary myofibroblastoma (MFB) is a rare benign tumor of the breast, with predilection for older men and postmenopausal women. When present in other sites, the term mammary-type or extramammary MFB (EMFB) is considered, and its diagnosis can be challenging. MFB variants include fibrous, cellular, infiltrative, myxoid, deciduoid, epithelioid, lipomatous and atypical. To date, only eight intraoral EMFB cases have been reported. A 64-year-old man presented with a pedunculated nodule on the buccal mucosa. After excisional biopsy, microscopy revealed a predominant myxoid proliferation, containing spindle cells, small and thin vessels, permeated by numerous mast cells. By immunohistochemistry, vimentin, CD10, CD34, CD99, bcl-2, desmin, and AR were positive. INI1 and Rb expression was intact. Ki-67 highlighted rare cells, whereas pan-CK, α-SMA, EMA, p63/p40, S100, SOX10, STAT6, CD68, myogenin, ER, and PR were negative. Currently, the patient is well. To our knowledge, this is the first case report of intraoral myxoid EMFB.
