Resumo: Cancer treatment often requires extensive tumor resections, leading to significant defects that compromise both function and aesthetics. This case demonstrates an innovative surgical technique for midface and distal orbital reconstruction using a microvascularized fibula flap (MFF). This approach restores bone structure and promotes functional recovery. Osteotomies were performed on the MFF, which was fixed with 1.5 mm miniplates and titanium screws at the recipient site. The first fibular segment was employed to reconstruct the distal orbital region, while the second segment repaired the orbital floor. The third segment was discarded, and the fourth was positioned in the alveolar ridge. This versatile technique allowed the first segment to act as a buttress at a 90° angle to the orbital floor, ensuring structural stability. The reconstruction yielded excellent functional outcomes, including improvements in mastication, phonation, and aesthetics, significantly aiding in the restoration of the patient's facial structure and contributing to their rehabilitation.
