Resumo: Verruciform xanthoma is a rare, benign, reactive lesion of the oral mucosa. A 55-year-old male presented with an erythroleukoplakic plaque measuring 0.8 cm on the left lower lip mucosa. He had previously undergone surgical resection of a squamous cell carcinoma close to the region, followed by radiotherapy. The patient is currently on hemodialysis three times per week. An incisional biopsy was performed to evaluate the lesion. Histopathological analysis revealed parakeratinized stratified squamous epithelium with a verrucous surface, neutrophilic exocytosis, focal basal layer hyperplasia, and keratin plugs in thickened parakeratinized areas. Acanthosis and elongated, occasionally anastomosing rete ridges were present, along with a chronic inflammatory infiltrate in the subepithelial connective tissue. Large foamy cells with round nuclei were confined to the connective papillae of the lamina propria, consistent with verruciform xanthoma. The patient is currently awaiting stabilization of renal function to undergo complete surgical removal of the lesion.
