Resumo: To prevent oral cancer through early detection, education, and risk factor assessment, 33.154 individuals were examined from 1989 to 2023 across 210 of 399 municipalities in Paraná, Brazil. From 2019 to 2023, 3.472 individuals in 33 municipalities underwent clinical assessments and completed questionnaires on demographics, habits, and healthcare access. Most participants were female (52.2%) and caucasian (74.9%), with 64.1% earning up to two minimum wages. Education levels: 46.9% incomplete primary, 26.7% secondary, 16.6% higher education, and 3.8% illiterate. Regarding dental care, 17.9% never attended and 30.6% visited annually. Alcohol and tobacco use were reported by 28.7% and 33.6%, respectively. Oral hygiene was regular or poor in 62.6%, and 67.7% had never received preventive care. Lesions were found in 543 individuals (15.6%) aged 41-70: 5.8% inflammatory, 8.8% traumatic, 3.8% actinic cheilitis, 1.0% leukoplakia, 0.8% papillomas, 0.8% lichen planus, and 1.2% suspected malignancies. These findings support targeted public health interventions.
