Resumo: The odontogenic keratocyst is a developmental cyst that, in most cases, affects the posterior region of the mandible. A 78-year-old male patient complained of a swelling in the posterior right maxilla. The occlusal radiographic examination did not allow the identification of the lesion due to the overlap of the maxillary sinus, the same occurred in the panoramic radiographic examination. An aspiration puncture was performed, obtaining yellow fluid. The material obtained through incisional biopsy was sent for histopathological evaluation, which revealed a cystic cavity lined by stratified parakeratinized squamous epithelium with few cellular layers. The basal layer was composed of cuboidal or columnar cells, with nuclei arranged in a palisade and a corrugated superficial layer. A flat interface was observed between the epithelium and the underlying connective tissue. The final diagnosis was odontogenic keratocyst. Initial treatment involved lesion decompression, and subsequently, enucleation surgery will be performed.
