Resumo: Carcinoma ex-Pleomorphic Adenoma (CXPA) is a rare malignant salivary gland tumor arising from a pre-existing pleomorphic adenoma. A 58-year-old female patient presented with a long-standing, asymptomatic submucosal nodule on the upper left labial mucosa. Histopathological examination of the biopsy specimen revealed a minor salivary gland malignancy composed of ductal and myoepithelial cells arranged in bilayered tubular structures within a hyalinized stroma. Immunohistochemical analysis demonstrated cytoplasmic CK7 positivity in luminal cells, while myoepithelial cells showed nuclear p63 positivity, cytoplasmic vimentin reactivity, and both nuclear and cytoplasmic S-100 expression. The CK7-positive cell population predominated, displaying marked nuclear and cytoplasmic pleomorphism with hyperchromatic and bizarre nuclei. These findings confirmed the diagnosis of CXPA. The patient is currently undergoing treatment and remains under close clinical surveillance. This case highlights the importance of thorough histopathological evaluation and immunohistochemical profiling in diagnosing this rare malignancy, particularly when it arises from minor salivary glands in unusual locations.
