Resumo: A 70-year-old female patient, former smoker, presented with a painful ulcer on the floor of the mouth lasting for two months, unresponsive to analgesics and antibiotics. Intraoral examination revealed a 10-mm crateriform ulcer covered by fibrinopurulent exudate in an area of soft tissue enlargement overlapping the healing abutment of a dental implant. An incisional biopsy was performed to rule out malignancy, and showed parakeratinized stratified squamous epithelium with ulceration and a dense mononuclear inflammatory infiltrate in the lamina propria, confirming the diagnosis of a traumatic ulcer. The floor of mouth enlargement was consistent with idiopathic hyperplasia of the sublingual glands. An intraoral protective device was fabricated to prevent the observed trauma. The patient initiated the prosthetic rehabilitation and remains under follow-up. This case highlights the complexity of oral rehabilitation in the presence of sublingual glands enlargement and reinforces the need for clinical and histopathological evaluation of persistent ulcers.
