Resumo: INTRODUCTION: It consists of the formation of stones in the ductal system of the salivary gland, with the submandibular gland being the most commonly affected. The precise etiology is unknown, and the stones are classified based on their size, presenting symptoms attributable to size, location and complications. CLINICAL CASE: Patient C.D.D., 61 years old, presented a progressively painful swelling on the left side of the neck for two years, which progressively increased over time. He sought care only when he had difficulty eating. There was a firm and sensitive edema in the left submandibular region on examination. On intraoral palpation, a marked edema was felt in the left Whartons duct, suggestive of sialolith. Removal was performed by incision over the Whartons duct. Biopsy confirmed the clinical diagnosis. The patient was discharged with oral antibiotics and mouthwash and was asymptomatic after two months of follow-up. Keywords: Sialolith, Whartons duct, Sialolithiasis.
