Resumo: An odontogenic keratocyst (OKC) is a benign intraosseous lesion, asymptomatic and locally aggressive with potential for recurrence. An 19-year-old patient came for a routine dental examination and was found in a panoramic radiography a well-defined, corticated, multilocular radiolucent image associated with the crown of the unerupted and distoangularly impacted left mandibular third molar. The radiolucency extends superoinferiorly from the mandibular notch to the angle of the left mandible, and anteroposteriorly from the third molar to the mandibular foramen. An incisional biopsy was performed under the hypothesis of OKC or dentigerous cyst, the microscopic analysis confirmed the diagnosis of OKC. The conservative treatment consisted of marsupialization of the lesion during 8 months, followed by extraction of the left mandibular third molar and lesion enucleation associated with cryotherapy. A 2-year follow-up showed significant healing of the bone without any sign of recurrence.
