Resumo: A 74-year-old female patient sought dental care due to a plaque-like lesion on the lower lip, described as whitish in color, with a rough surface and slightly indurated consistency, measuring approximately 10 mm at its greatest dimension. The lesion had been present for six months with reported progressive growth. Given the clinical suspicion of leukoplakia, an incisional biopsy was performed. Histopathological analysis revealed an epithelial neoplasm with predominantly endophytic growth, composed of neoplastic cells lining curved and branching sinus-like spaces, as well as keratin-filled crypts extending deeply into the lamina propria. The neoplastic cells exhibited moderate pleomorphism, mild to moderate anisocytosis and anisokaryosis, increased mitotic activity, dyskeratosis, and nuclear hyperchromatism. The lamina propria showed marked solar elastosis and a mild to moderate chronic inflammatory infiltrate. Based on these findings, the diagnosis of carcinoma cuniculatum was established, and the patient was referred for specialized treatment.
