Resumo: A 51-year-old female patient sought dental care for a mandibular cystic lesion. She reported previous treatment five years ago at another center, where she was diagnosed with a radicular cyst and underwent endodontic treatment, with no improvement. Clinically, an asymptomatic multilocular radiolucent lesion, circumscribed by a radiopaque halo, was observed at the apices of teeth 41 to 32, causing roots divergence. Cone-beam computed tomography revealed buccal bone plate discontinuity and cortical expansion. The main hypothesis was odontogenic cyst or tumor. Enucleation of the lesion was performed. Microscopically, a multilocular cystic cavities lined by stratified squamous or cuboidal epithelium, with papillary projections, intraepithelial microcysts, apocrine metaplasia, mucous cells, and satellite cysts were observed. The final diagnosis was glandular odontogenic cyst. After one-year follow-up, no recurrence was observed. This case highlights that some odontogenic lesions share clinical and microscopic features, emphasizing the importance of histopathological evaluation for an accurate diagnosis.Funding: CAPES (Code 001)
