Resumo: Extramedullary leukemic manifestations are an uncommon feature of chronic myelomonocytic leukemia (CMML), especially in the oral cavity. A 56-year-old female previously diagnosed with CMML-confirmed by myeloid infiltration in skin lesions-and treated with hematopoietic stem cell transplantation presented with disease relapse after five years. Oral examination revealed an ulcerated nodular lesion (1.5 cm) with indurated borders and yellowish membrane on the posterior tongue. Bilateral ulcerated lesions with raised, erythematous, indurated borders and pseudomembranous covering were also noted on the soft palate. Incisional biopsy was performed. Histopathological analysis revealed ulcerative chronic and acute inflammation with atypical cell proliferation. Immunohistochemical staining showed positivity for CD45, CD68, Ki67, and myeloperoxidase, supporting leukemic infiltration. These findings confirmed oral involvement by CMML. The patient received palliative care and died within 30 days. Although oral lesions are not specific to CMML, their presence in relapse may suggest severity and poor prognosis, highlighting prompt, interdisciplinary management.
