Título: Evaluation of Salivary Inflammatory Mediators in Oral Lichen Planus: A Case-Control Study

Resumo: Objective:
Oral lichen planus (OLP) is a chronic, inflammatory, and autoimmune disease. Although its etiology remains uncertain, its pathogenesis appears to be associated with salivary inflammatory mediators. This study aimed to evaluate the salivary concentration of inflammatory mediators in patients with OLP and healthy individuals.

Study Design:
This case-control study assessed salivary levels of pro- and anti-inflammatory mediators in patients with a clinical and microscopic diagnosis of OLP (case; n=31) and healthy individuals (control; n=23), and examined the association between these mediators and clinical parameters of disease severity. OLP patients were divided into severe cases (atrophic/erosive/ulcerated and symptomatic) and non-severe cases (reticular/plaque and asymptomatic). Salivary levels of cytokines IL-1, -2, -6, -8, -17, -4, -10, IFN-γ, and TNF-α were measured using the Bead Cytometry Assay, while nitrite and MPO were analyzed using the Griess colorimetric method.

Results:
The salivary concentration of IL-6, -10, and nitrite was higher in OLP patients compared to the control group. Additionally, nitrite levels were elevated in patients with more pronounced symptoms (p<0.05). Severe OLP cases showed higher concentrations of IL-17, -2, -4, and MPO than the control group (p>0.05).

Conclusion:
The findings suggest that IL-6, -10, and nitrite may serve as biomarkers of OLP severity and contribute to its pathogenesis and symptomatology.
