Resumo: A peripheral manifestation of an odontogenic keratocyst (OKC) is a rare occurrence. We report the case of a 43-year-old man who presented with a yellowish gingival nodule located between teeth 33 and 34. A periapical radiograph revealed no evidence of intraosseous involvement. During the initial biopsy, a translucent fluid was noted, and the clinical diagnosis was an adult gingival cyst. Microscopically, although no epithelial lining was identified, eosinophilic lamellae interpreted as keratin layers were observed. Two months later, the lesion recurred, displaying the same clinical characteristics. A second biopsy was performed, during which a small, pear-shaped bone cavity was discovered between the roots of teeth 33 and 34 and was subsequently curetted. Following the procedure, a new periapical radiograph revealed a radiolucent area between the aforementioned teeth, which was further confirmed by computed tomography. Histopathological examination demonstrated the classical microscopic features of an OKC. The patient remains under clinical follow-up.
