Resumo: Histoplasmosis may resemble other oral infections or neoplasms, making diagnosis challenging. We report three cases with oral involvement. Case 1: a 49-year-old HIV-positive white male with an ulcer on the buccal gingiva of tooth 36 and multiple lesions on the maxillary ridge and right hard palate (40-day evolution). Case 2: a 52-year-old white male with a painful ulcer on the tongue tip (60 days). Case 3: a 48-year-old white male with an ulcer on the lower labial mucosa and multiple ulcers on the left buccal mucosa (1-month evolution). Differential diagnoses included paracoccidioidomycosis, histoplasmosis, and squamous cell carcinoma. Incisional biopsies revealed granulomatous inflammation and small PAS-positive yeasts, confirming histoplasmosis. All patients were referred to infectious disease specialists and remain under follow-up. These cases underscore the importance of dental professionals in recognizing oral signs of systemic infections, enabling early diagnosis and appropriate referral.
