Resumo: Cardiac Amyloidosis (CA) is a rare and difficult to diagnose disease characterized by abnormal extracellular deposition of amyloid in various tissues and organs. It carries a poor prognosis with a survival rate of less than six months without treatment. Case report of a 62 year old male patient presenting with significant dyspnea, dysphagia, and a clinical suspicion of CA. Physical examination revealed swelling in the upper and lower limbs and abdominal region, along with an enlarged, depapillated tongue that changed color upon palpation. Tongue's biopsy was performed, and histopathological analysis pseudoepitheliomatous with hyperplasia, connective tissue with no inflammatory, but with hyaline material deposits in the interstitium and in the walls of small vessels. Congo red staining was positive, and the material displayed birefringence under polarized light, confirming the presence of amyloid. The specimen was analyzed by mass spectrometry, which confirmed the diagnosis of CA. The patient was referred for appropriate treatment
