Título: CLINICOPATHOLOGICAL ANALYSIS OF WHITE PLAQUES DIAGNOSED AS LEUKOPLAKIA AND ORAL LICHEN PLANUS: A 10-YEAR CASE SERIES

Resumo: Objective: To analyze white plaques clinically and microscopically diagnosed as oral leukoplakia(OL) and oral lichen planus(OLP).
Study Design: A retrospective, cross-sectional, descriptive study. Histopathological reports and medical records from 2009 to 2018 were reviewed for patients with clinicopathological diagnoses of OL and OLP. Data on gender, age, presence of reticular pattern, symmetry, multifocality, anatomical location, and grade of oral epithelial dysplasia(OED) were tabulated.
Results: A total of 204 patients and 315 lesions were analyzed. Most patients were female(approximately 65%) and aged 50-60 years. OL was more frequent, with an average of 26.9 lesions/year, while OLP accounted for 7.2 lesions/year. Simultaneous diagnoses of OL and OLP were observed in 3.9% of patients. The most affected regions were the tongue and buccal mucosa. Multifocal and asymmetrical lesions predominated, with reticular patterns infrequent. Most lesions presented OED, predominantly mild to moderate, and more commonly observed in OL.
Conclusion: OL and OLP share demographic(gender and age), clinical(white plaque and location), and morphological(hyperkeratosis and OED) profiles. OL presents a higher risk of malignant transformation. Moreover, the multifocality of OL, associated with the analyzed features, suggests the need for effective diagnostic criteria for proliferative verrucous leukoplakia, a challenging entity due to persistence, treatment resistance, and high malignancy rate.
