Título: IMMUNOHISTOCHEMICAL PROFILE OF KI-67, 8-OHDG, BAX, AND BCL-2 IN ORAL LEUKOPLAKIA IN PATIENTS ≤40: COMPARISON WITH OSCC

Resumo: Objective: To evaluate the role of oxidative DNA damage (8-hydroxy-2′-deoxyguanosine [8-OHdG]), cell proliferation (Ki-67), and apoptosis (Bax and Bcl-2) in oral leukoplakia (OL) and oral squamous cell carcinoma (OSCC) in patients aged 40 years or younger.
Study Design: Sixty-four OL, twenty-eight OSCC, and ten normal oral mucosa (NOM) samples were analyzed through immunohistochemistry (IHC) for 8-OHdG, Ki-67, Bax, and Bcl-2 expression. Immunoreactive score (IRS) was used for 8-OHdG, Bax, and Bcl-2, and labeling index (LI) for Ki-67. Mann-Whitney and Kruskal-Wallis tests were applied to compare marker expression and clinicopathological correlations.
Results: A significantly higher Ki-67 index was observed in OSCC compared to OL (p=0.001). No significant differences were found in 8-OHdG, Bax, or Bcl-2 expression between groups. However, OL cases with severe dysplasia showed higher 8-OHdG levels than those with mild or no dysplasia (p=0.039). Smaller OL lesions exhibited higher Bax expression (p=0.035).
Conclusion: Findings reinforce Ki-67 as a relevant proliferation marker in OL progression among young individuals. While oxidative stress and apoptosis markers were not associated with OSCC, their expression patterns suggest a potential role in early dysplastic changes and lesion size, indicating possible involvement in early carcinogenesis.
