Resumo: ABSTRACTINTRODUCTION: Pemphigus vulgaris is an autoimmune disease that, if left untreated, can lead to death. Oral lesions are often the first clinical manifestation. The disease is characterized by blister formation due to the abnormal production of autoantibodies directed against epidermal cell glycoproteins-desmoglein 3 and 1-which are components of desmosomes. These autoantibodies bind to them, inhibiting the molecular interaction responsible for cell adhesion. As a result of this autoimmune attack on desmosomes, a cleft forms within the epithelium, leading to the development of an intraepithelial blister. Clinically, patients present with painful erosions and irregular ulcers randomly distributed across the oral mucosa. CLINICAL CASE: Patient R.R.R., 33 years old, was diagnosed 10 months ago after the appearance of wounds on the body, including in the mouth. The patient reported painful and erosive lesions.
