Título: FUSARIUM INFECTION IN A PATIENT WITH A HISTORY OF MYELODYSPLASTIC SYNDROME: A CASE REPORT

Resumo: A 74-year-old female patient with history of thyroid cancer and treatment-related myelodysplastic syndrome, undergoing chemotherapy with Decitabine, developed profound neutropenia and thrombocytopenia. After 22 days of hospitalization, she reported oral pain. Clinical examination revealed edema and necrotic ulcerative lesion on the maxillary gingiva surrounding dental implants. Blood cultures were negative. Simultaneously, she presented fever and hyperemia in the left hand, suggestive of infectious process. Biopsy of the hand lesion revealed hyphae and yeast. Empirical treatment with voriconazole was initiated; however, the patient showed no clinical improvement, progressing to sepsis. Incisional biopsy and scraping of the oral lesion were then performed, proving essential for diagnostic clarification. Histopathological analysis confirmed Fusarium infection, prompting a change in therapy to Amphotericin B. This case underscores the critical role of oral examination and biopsy in immunocompromised patients, as the oral lesion was key to identifying the invasive fungal infection and guiding timely and appropriate treatment.
