Título: Oportunistic infections in a patient with Erythema Multiforme: A Case Report

Resumo: A 79-year-old female patient presented severe oral pain lasting one month, unresponsive to previous treatments, weight loss, and inability to perform oral hygiene. Clinical examination revealed extensive ulcerated lesions on buccal and lower labial mucosa, with fetid odor. Initial management included local disinfection with chlorhexidine, antimicrobial photodynamic therapy, and Prednisone 20 mg/day. An incisional biopsy was performed, confirming erythema multiforme. At five-day follow-up, partial clinical improvement was noted. However, by day eight, the patient worsened, became unable to ingest liquids, and developed punctate palatal lesions suggestive of secondary herpes infection. She was referred to the emergency service for supportive care. In collaboration with the medical team, corticosteroid therapy was intensified, increasing Prednisone to 60 mg/day. During follow-up, pseudomembranous candidiasis was also diagnosed as a co-infection. This case highlights the risk of opportunistic infections in elderly, immunocompromised patients with erythema multiforme undergoing systemic corticosteroid therapy and the importance of interdisciplinary management.
