Título: DIRECT ORAL MICROSCOPY IN THE ASSESSMENT OF PROLIFERATIVE VERRUCOUS LEUKOPLAKIA WITH MALIGNANT TRANSFORMATION

Resumo: The aim of this study was to report the use of direct oral microscopy in evaluating a case of proliferative verrucous leukoplakia (PVL) with malignant transformation. An 83-year-old female patient was under follow-up for 5 years due to PVL with epithelial dysplasia ranging from moderate to severe. Clinical examination showed multiple white plaques with a verrucous surface, ill-defined borders, and subtle erythematous areas on the anterior mandibular gingiva and left buccal mucosa. Lesions were assessed under magnification (operating microscope), allowing visualization of erythematous areas not seen clinically, variations in surface thickness and texture, and improved delineation of lesions' peripheral limits. Microscopy guided the biopsy site selection, and the incisional biopsy confirmed squamous cell carcinoma. The patient was referred to surgical oncologic treatment. Direct oral microscopy enhances the detection of subtle surface alterations and may serve as a valuable tool in the monitoring and early identification of malignant progression in PVL.
