Resumo: Objective: The aim of this study was to evaluate the feasibility of diagnosing oral leukoplakia using saliva samples analyzed with Artificial Intelligence (AI) algorithms and infrared spectroscopy. We also aimed to achieve a fast and smart diagnosis using the proposed technology for early detection.Study Design: Saliva samples were collected from 30 patients without oral lesions (control group) and 30 patients with oral leukoplakia (experimental group) using a Bruker spectrometer equipped with an ATR-FTIR accessory. Spectra were acquired with 32 scans and a spectral resolution of 4 cm⁻¹. The samples were analyzed within the FTIR fingerprint region (900-1800 cm⁻¹), and the main biochemical constituents of saliva were identified. Saliva classification models were developed using AI algorithms.Results: Spectroscopic analysis revealed distinct molecular signatures, with significant bands associated with phosphodiesters (900-1000 cm⁻¹), protein vibrations and lipid markers (1500-1600 cm⁻¹), and fatty acid esters (~1700 cm⁻¹). Unsupervised statistical analysis implemented in Python demonstrated robust classification performance using three machine learning models: Extra Trees Classifier, Random Forest Classifier, and SVM, each achieving accuracy greater than 80%.Conclusion: Infrared spectroscopy combined with AI algorithms represents a promising tool for the diagnosis of oral leukoplakia and may contribute to the early detection of oral cancer.
