Resumo: Cystadenoma is a rare benign epithelial salivary gland neoplasm, characterized by a predominantly multicystic growth pattern. The lesion represents 1 to 4% of all salivary gland neoplasms. About half of cases occur in the parotid gland, followed by the minor glands of the lip, buccal mucosa, palate and tongue. The floor of the mouth is rarely involved. We report a case of a 51-year-old man presenting a asymptomatic swelling in the right sublingual caruncle with unknown evolution. Physical examination revealed a smooth, red mass with a flaccid consistency, measuring approximately 5 mm. Under the clinical diagnosis of salivary duct cyst, an excisional biopsy was performed. Microscopic examination showed multiple cystic spaces lined by oncocytic epithelium, exhibiting intraluminal papillary proliferation, intraepithelial microcysts, and mucous, squamous, and apocrine cell metaplasia. Therefore, the histopathological diagnosis was salivary gland cystadenoma. The patient is currently under follow-up with no signs of recurrence.
