Resumo: Lateral periodontal cyst (LPC) is a rare developmental odontogenic cyst that accounts for less than 1% of all oral and maxillofacial cysts. Botryoid odontogenic cyst (BOC) is a rare, more aggressive polycystic variant of LPC. LPC and BOC are characterized by a radiolucency between the roots of erupted vital teeth, especially mandibular premolars, and are rarely associated with impacted teeth. We report an atypical case of a 41-year-old woman diagnosed with BOC during a routine radiographic examination, which revealed a well-defined hypodense image associated with the crown of impacted tooth 48. Under the provisional diagnosis of dentigerous cyst, enucleation followed by curettage was performed, along with extraction of 48. Microscopic examination showed multiple cystic cavities lined by thin epithelium with focal plaque-like thickenings, exhibiting a swirling pattern and occasional clear cells. Therefore, the histopathological diagnosis was BOC. After 6 months of follow-up, there are no signs of recurrence or complications.
