Resumo: A 51-year-old male patient, HIV-positive with AIDS, thrombocytopenic, and hepatopathic, was admitted to a tropical diseases reference hospital for management of immunosuppression, presents multiple oral lesions on the dorsal tongue with a hairy appearance and coloration alternating between blackish and yellowish-white, as well as whitish ulcerations with erythematous margins on the tongue and on the buccal mucosa, the latter exhibiting markedly elevated margins, an irregular surface, and a slightly hardened consistency. No history of drug abuse. An incisional biopsy using a punch was performed on the left lingual body due to suspicion of malignancy. Histopathological examination revealed disorganized connective tissue with dense, diffuse lymphohistiocytic infiltrate, accompanied by numerous small yeast-like structures that stained positive with PAS and Gomori-Grocott, consistent with the micronidia of Histoplasma capsulatum, establishing a diagnosis of progressive disseminated histoplasmosis. Patient began systemic antifungal therapy, showed clinical improvement and remains under follow-up of 5 months to date.
