Resumo: Osteochondromas are benign osteocartilaginous proliferations arising from endochondral ossification, rarely occurring in craniofacial bones, with few reports involving the mandibular condyle or posterior maxilla. This study presents an exceptionally rare case of a peripheral osteochondroma in the anterior maxilla, a previously undocumented location.A 66-year-old female presented with a painless, pedunculated, firm nodule in the maxillary midline, evolving over 18 months. The lesion was surgically excised with clear margins. Histopathological analysis revealed a well-circumscribed mass beneath the mucosal epithelium, featuring stellate bi- and trinucleated cells within a basophilic myxoid matrix, alongside peripheral chondroid and osteoid tissue-confirming the diagnosis. To our knowledge, this is the first reported case of peripheral osteochondroma in the anterior maxilla. The findings underscore the importance of including this entity in the differential diagnosis of firm, pedunculated oral lesions, even in atypical sites. Surgical excision with clear margins remains the gold standard for treatment.
