Resumo: Pediatric squamous cell carcinoma (SCC) is extremely rare, accounting for less than 1% of head and neck malignancies in children. A 9-year-old male patient was referred for evaluation of a painful, crateriform, dome-shaped nodular lesion with hardened borders on the midline of the lower lip semimucosa. The lesion had persisted for 1 month. The patient and his family reported no risk factors. Given the clinical differential diagnosis of keratoacanthoma, primary syphilis, or SCC, an incisional biopsy was performed. Histopathological analysis revealed a neoplastic proliferation of well-differentiated epithelial cells invading the connective tissue, along with pleomorphism and atypical mitotic figures. Based on these findings, the diagnosis of SCC was established. The patient was referred to surgical treatment and systemic evaluation by a medical team. The lesion was completely resected, and the diagnosis of SCC was confirmed in the surgical specimen. After seven months of follow-up, the patient remains free of disease.
