Título: PROGNOSTIC VALUE OF COPY NUMBER ALTERATIONS IN THE MALIGNANT TRANSFORMATION OF ORAL LEUKOPLAKIA: A SYSTEMATIC REVIEW AND

Resumo: Objective: To evaluate the role of copy number alterations (CNAs) in predicting malignant transformation (MT) of oral leukoplakia (OL).
Study Design: A systematic search was conducted across five databases and gray literature. Risk of bias was assessed using the National Heart, Lung, and Blood Institute (NHLBI) quality assessment tools. Data synthesis included a multilevel logistic regression, and a random effect meta-analysis using aggregated study data.
Results: Six studies met the eligibility criteria. Two studies were included in the logistic regression and four studies in the meta-analysis. Regarding the regression, the presence of CNAs increased the risk of malignancy by 5.55 times (OR=5.55; 95% CI[1.84-16.78; p=0.002]). Genomic losses conferred a 10-fold higher risk of MT (OR=10.00; 95% CI[1.70-58.88; p=0.009]), while gains showed a 4.35-fold; (OR=4.35; 95% CI [1.35-13.99; p=0.009]). In the meta-analysis, the presence of CNAs was significantly associated with the MT of OLs (RR 2.93 [95%CI=1.48-4.38]). Risk of bias in the studies was moderate to high.
Conclusion: This systematic review emphasizes the prognostic importance of CNAs in OL, highlighting its potential as a tool for risk stratification.
