Resumo: Oral lichen planus (OLP) is a chronic mucocutaneous disease with malignant potential. We report the case of a 38-year-old man diagnosed with reticular and erosive OLP affecting the buccal mucosa and skin. The patient was maintained under six-month follow-up, returning mainly during symptomatic episodes. The lesions remained clinically stable and were treated with topical corticosteroids when symptomatic. After 25 years of follow-up, he reported persistent pain in the left buccal mucosa, where an atrophic lesion was biopsied and revealed OLP with severe epithelial dysplasia. One year later, a new biopsy at the same site, confirmed squamous cell carcinoma. The patient underwent surgical resection and radiotherapy; however, regional metastases developed, and he died within a year. This case highlights the importance of long-term monitoring, early recognition of high-risk lesions, and strict documentation, even in cases with prolonged clinical stability.
