Resumo: Objective: To identify structural and organizational barriers that impact the early diagnosis of oral cancer in Primary Health Care(PHC).Study Design: A cross-sectional, descriptive, and analytical study conducted through a questionnaire applied to dentists working in PHC. Variables related to service organization, matrix support, and educational actions were investigated.Results: With a total of 248 participants, the data reveal a lack of awareness among professionals regarding the structure and resources available in their municipalities for the early diagnosis of oral cancer. More than half were unaware of educational actions on the subject(55.24%), the existence of specialized matrix support(64.52%), the referral flow for biopsy materials(49.19%), and the availability of in-house or partnered laboratories for histopathological/anatomopathological analysis(54.44%). A statistically significant association was observed between health region and availability of matrix support(p<0.001) and laboratory partnerships(p=0.007), highlighting regional disparities in care organization.Conclusion: The absence of defined protocols, fragmentation of the healthcare network, and uneven distribution of technical support compromise the effectiveness of PHC in the early diagnosis of oral cancer. These findings may contribute to the development of a care pathway that takes into account the regionalization of services, ensuring equitable access to diagnosis while respecting the specificities of each territory.
