Resumo: Background: The differential diagnosis between Oral Leukoplakia (OL) and Proliferative Verrucous Leukoplakia (PVL) is challenging due to the similarity in their clinical manifestations. There is still no consensus in the literature regarding the average time required to establish a differential diagnosis between these two lesions. Objective: This study aimed to improve the differential diagnosis between OL and PVL by identifying sociodemographic and clinicopathological variables that influence the distinction of these lesions. Study Design: This is a cross-sectional and observational study, including 131 patients, comprising 95 cases of OL and 36 cases of PVL. Statistical analysis used binomial logistic regression and ROC curve to evaluate the optimal follow-up period for differential diagnosis. Results: The results highlighted epithelial dysplasia (p = 0,015), smoking (p=0,048), and follow-up duration (p=0,001) as critical factors for distinguishing OL from PVL, indicating that a follow-up period between 2.7 and 5.5 months significantly increases diagnostic accuracy. Conclusion: Standardized follow-up is essential for a reliable differential diagnosis between OL and PVL.This research was supported by Proex-UFES and CAPES.
