Título: Chondrosarcoma Grade III In The Mandibular Alveolar Mucosa: A Rare Case With Agressive Clinical Presentation

Resumo: A 60-year-old male presented with a painful, nodular lesion in the right mandibular alveolar mucosa. Clinical and radiographic examinations revealed a firm, reddish mass with areas of cortical bone discontinuity and hypodensity on CT imaging, raising suspicion of an aggressive neoplasm. Incisional biopsy showed a malignant neoplasm composed of pleomorphic, round to oval cells with hyperchromatic nuclei arranged in chondroid-like islands. Immunohistochemistry revealed focal positivity for S100 and D240, high Ki-67 proliferative index (80-85%), and negativity for AE1/AE3, HBME-1, SOX10, 1A4 (smooth muscle actin), and desmin. These findings supported the diagnosis of high-grade chondrosarcoma (Grade III). Due to its rarity in the oral cavity, especially in the soft tissue and alveolar mucosa, diagnosis was challenging and required integration of histopathological, immunohistochemical, and imaging data. This case underscores the importance of early biopsy and multidisciplinary planning for rare oral sarcomas, given their aggressive behavior and poor prognosis.
