Resumo: A 72-year-old leucoderm female presented with a painful ulcerated lesion on the soft palate for approximately five months, along with feeding difficulties and unintentional weight loss. Clinical examination revealed a 3 cm ulcerated, infiltrative lesion with elevated, indurated borders and no signs of healing. Incisional biopsy confirmed moderately differentiated invasive squamous cell carcinoma (SCC). Imaging (CT and cervical ultrasound) showed suspicious lymphadenopathy in left level II. The patient underwent wide resection of the soft palate via mandibulotomy, followed by osteosynthesis with a 2.0 fixation system and selective neck dissection. Anatomical and functional reconstruction was achieved through a multidisciplinary approach, and adjuvant radiotherapy was administered. The postoperative course was uneventful, and the patient remains under oncologic follow-up with a favorable response and no signs of recurrence. This case highlights the surgical and diagnostic complexity of soft palate SCC and reinforces the importance of integrated, early intervention for optimized treatment outcomes.
