Resumo: A 26-year-old male patient has presented with a complaint of anosmia, epistaxis, and nasal obstruction for a duration of eight years. Magnetic resonance imaging revealed a polypoid lesion in the lower left portion of the nasal cavity, with well-defined lobular contours measuring 5,0 x3,0 x7,2 centimeters. No extension to the orbit or intracranial structures was observed. Cerebral angiography revealed no aneurysms or arteriovenous malformations, indicating that no preoperative approach was necessary. Subsequently, an excisional biopsy was performed, and histopathological analysis revealed the presence of blood vessels of various sizes, a fibrous stroma, and areas of ciliated pseudostratified columnar epithelium. The immunohistochemical examination yielded positive results for the androgen receptor, beta-catenin, and CD34, which favored the diagnosis of angiofibroma of the sinonasal tract. The patient is currently undergoing follow-up seven months after surgery, with no clinical or imaging signs of recurrence, a patent airway, and no bleeding.
