Título: CONSERVATIVE MANAGEMENT OF OSTEORADIONECROSIS: A CASE REPORT

Resumo: A 60-year-old male with a history of tongue squamous cell carcinoma (2019), treated with surgery, chemotherapy and radiotherapy, presented with persistent right mandibular pain. He had undergone prior dental extractions, was a former smoker, and had no comorbidities. Clinical and radiographic evaluations revealed bilateral bone exposure, root remnants, and mandibular fractures. He was on ciprofloxacin and fluconazole for extraoral suppuration. Conservative treatment included antibiotics, bone sequestra removal, debridement, extractions, high-power defocused diode laser was used for decontamination (GEMINI EVO, 810 + 980 nm, 1W), photobiomodulation (GEMINI EVO 1W/30J extraoral; DMC Therapy 0.1W/4J intraoral) and antimicrobial photodynamic therapy (methylene blue 0.01% MMOptics Laser DUO 0.1W/5J). After three months of weekly follow-ups, fistula closure, partial mucosal and bone repair were observed. Microbiological analysis showed a reduction in Streptococcus oralis (4.49 to 3.5) and Staphylococcus aureus (4.41 to 0.00) after laser decontamination. This multidisciplinary approach was effective in promoting clinical and symptomatic improvement.
