Título: HERPES SIMPLEX VIRUS TYPE 1 REACTIVATION IN HOSPITALIZED COVID-19 PATIENTS: CLINICAL AND EPIDEMIOLOGICAL IMPLICATIONS

Resumo: Objective: Investigate the presence of HSV-1 in saliva and oral lesion samples from hospitalized COVID-19 patients, assessing its association with clinical and epidemiological variables. Study Design: Saliva samples from 120 hospitalized COVID-19 patients were analyzed by RT-PCR for SARS-CoV-2 and HSV-1 detection. Oral lesion samples from 11 patients underwent Papanicolaou staining to identify HSV-1-related cytopathic effects. Results: HSV-1 was detected in the saliva of 15 patients (12.5%), with significantly higher prevalence among individuals aged 60 or older (p = 0.0061), those with severe COVID-19 (p < 0.001), and patients presenting oral lesions (p < 0.001). HSV-1 detection was associated with adverse clinical outcomes, including increased mortality. All patients with HSV-1 in oral lesions also showed viral presence in saliva, although the inverse was not consistently observed. Cytological analysis confirmed a strong association between HSV-1 and cytopathic effect (p < 0.001), indicating that reactivation is likely linked to immune dysfunction in severe COVID-19 cases. Conclusion: HSV-1 reactivation may play a role in clinical deterioration of hospitalized COVID-19 patients. Saliva testing offers a non-invasive approach for early detection, potentially aiding clinical management. Further studies are needed to better understand the immunological mechanisms underlying HSV-1 and SARS-CoV-2 interactions. Study funded by FAPESP.
